Equipment Release Form

Francis Tuttle Technology Center – Danforth campus students are allowed to use DSLR cameras, MacBooks, iPads, drawing tablets, and much more equipment during the class period and off the premises for academic/educational purposes that fulfill the requirements and responsibilities of the program.

All equipment is strictly the property of Francis Tuttle Technology Center. Equipment checked out for use during the class period should be returned and put away properly at the end of the class period. Due to limited equipment, items may only be checked out over one night (the weekend is considered one overnight), but the student may check out equipment as often as needed, if available. Students must be in good standing in the class to check out equipment over one night. Good standing means no orange on the students grade sheet at the time of check out.

Upon check-out, the student is responsible for inspecting equipment for issues, missing items, & damages to limit liability. The number of items checked out at one time may be limited to what is reasonable/fair at the instructor’s discretion. The student is responsible for all equipment, and must be returned in the same working order as when borrowed. Equipment is checked out to use outside of class time by the instructor and is to be returned to and signed in by the instructor. Students must sign an Equipment Release Form to reserve and check out any equipment.

Minor offenses such as returning equipment late, failing to report broken equipment, or other offenses made at the instructor’s discretion will be subject to a 3-Strike Policy:

  • Strike 1: Warning.
  • Strike 2: Prohibited from using equipment for 1 month.
  • Strike 3: Prohibited from using equipment the rest of the semester.

You agree to accept liability for the equipment checked out through our online inventory by signing below. It is understood that should any damages or loss of the equipment occur while it is in your possession, you and your parents or guardian will be held fully responsible and liable for the full amount of repairs/replacement of the equipment.

EQUIPMENT CHECKOUT CONTRACT

I agree to assume financial responsibility for the equipment. I will pay for any repairs due to negligence when the equipment is in my control. I also agree to pay for or replace with comparable equipment and any items lost or stolen while in my care. I understand that my grades, transcript, and completion certificate will be held until this debt is paid.
